We invite you to share your experiences and insights in the 2025 Community Health Survey. Your feedback drives real change. By participating, you help identify key health challenges and guide solutions that improve the well-being of our community. The results will help local organizations, policymakers, and healthcare providers address the most pressing health needs in Monterey County.
Who We Are
The Monterey County Health Needs Collaborative is a partnership of hospitals, health organizations, and community agencies working together to improve the health and well-being of Monterey County residents. Originally founded in 2022, the Collaborative includes the County of Monterey Health Department, Mee Memorial Healthcare System, Montage Health, Natividad, Salinas Valley Health, and United Way Monterey County. In 2025, California State University Monterey Bay (CSUMB), and Central California Alliance for Health (Alliance) have joined the collaborative.
By combining expertise, resources, and community engagement, the Collaborative strives to identify and address the most pressing health challenges facing our region. Through initiatives like the Community Health Needs Assessment (CHNA), we broaden our partnership and leverage our resources to meet common health goals
Not-for-profit hospitals must complete a CHNA every three years and Health Departments must do one every five years. By coming together, the hospitals, Health Department, and other community stakeholders will be able to better collaborate to prioritize and address the county’s most pressing health needs. The 2025 CHNA consists of both primary and secondary data including county level data, census data, vital statistics, online key informant surveys, customized local health surveys and other health related data.
Why Your Participation Matters
The 2025 Community Health Survey is a critical tool in understanding the health needs of Monterey County residents. The information gathered will help:
- Identify priority health concerns, such as mental health, chronic disease, access to care, and nutrition.
- Inform hospitals, public health agencies, and local organizations on where to focus resources and services.
- Support the development of health programs, funding opportunities, and policy changes that directly benefit our community.
By sharing your experiences, you play an essential role in shaping the future of health in Monterey County. Your responses will be kept confidential and will contribute to a data-driven approach in addressing local health challenges.
What Happens Next?
After data collection is complete, the Monterey County Health Needs Collaborative will analyze the survey results alongside other health data sources. The findings will be used to:
- Develop CHNA to guide health initiatives for the next three years.
- Strengthen collaboration between hospitals, public health agencies, and community organizations.
- Implement targeted programs and interventions to improve health outcomes based on identified needs.
The final CHNA report will be made available to the public, ensuring transparency and continued community engagement in the process.

2022 CHNA Results
The public gained access to the overall CHNA for Monterey County, along with individual hospital CHNA reports that provided a more focused assessment of primary and secondary zip codes within each hospital’s service area. Following the report’s release, health care systems and community partners analyzed the data and developed implementation strategies to address key health needs, leveraging existing strengths and resources to improve health opportunities and services for individuals and the community as a whole.
These efforts, alongside strengthened partnerships between hospitals, public health agencies, and local organizations, contributed to measurable improvements in community health outcomes. The CHNA findings continue to guide future initiatives, ensuring that Monterey County remains focused on addressing the most pressing health challenges facing its residents.
